Pressure ulcers present a significant health care threat to patient with restricted mobility or chronic disease and to older patients. Pressure is major causative factor in pressure ulcer formation. However, several factors play a role in determining whether pressure is sufficient to create tissue ischemia proceeding to tissue death. Pressure ulcers that have been subjected to shear often are first seen with undermining. Ultimately the dressing should be focused on maximizing healing of pressure ulcers through infection control, sharp and autolytic debridement, clean and moist condition. All of them are a challenge for both the patient and the health care provider. We report a case of a 85-year-old male patient who developed sacral pressure sore due to long term immobilization after brain surgery.
